Chapter Historical Reports

Each chapter of Omicron Tau Theta is to submit to the National Historian no later than October 15 of each year a report of its chapter activities, member initiates, honorary members, chapter OTT awards, OTT chapter officers, national OTT awards, National OTT officers, and National Directory information.

The reports should represent chapter activities from December to December each year. Therefore, any proposed activities to be held between October 15 and December 31 should be listed in the yearly report. Likewise, if new members are to be initiated between October 15 and December 31, they should be listed as new members.

The officers listed are those in office at the time the report is prepared. If the chapter's fiscal year is based on a calendar year, please give the name and address of the advisor, president, and National delegate and alternate, effective January of the following year. This information is used to update the National Directory and coincides with the fiscal year of National Omicron Tau Theta. If a chapter has more than one advisor, please decide the one advisor who is to be listed in the National Directory and to which all National correspondence should be directed.

All reports should be camera-ready. To maintain uniformity in the presentation of the reports and to improve the appearance of the National Historical Yearly Report, each chapter is asked to follow the format guide given on the following page.

FORMAT GUIDE

Use chapter Omicron Tau Theta letterhead or, if none, university letterhead for the first page. Use plain paper for succeeding pages. Please make sure the name of the university and local chapter is on the front page of the report.

Use a 1.25 left and right margin.

Use 10 pitch (12 point) type size.

Single space the report with double-spacing between paragraphs or enumerated listings.

Begin the first-page heading two or three spaces below the letterhead; space 3 times after the heading before typing the body of the report.

Leave at least a 1-inch bottom margin.

Provide a heading at the top of each page, typed at the left margin, beginning on line 7.

See the sample report on the following pages and use it as a guide to develop and prepare the yearly chapter report.

Send the original and three copies of the report to the National Historian by October 15. The Historian sends one copy to the National President, one copy to the National Secretary, one copy to the office of the Executive Director, and saves one copy for the succeeding historian.

Each chapter's delegate to the National Council's annual meeting at AVA should bring enough copies of the OTT Chapter Yearly Historical Report for each member of the National Council (about 30 copies).
